“Late pregnancy isn’t a barrier to motherhood, but it does call for careful planning,” she explains. Women are born with a finite number of eggs, and both the quantity and quality naturally decline with age—more noticeably after 35, and rapidly beyond 40.

Dr. Bhatt encourages women to schedule early fertility evaluations that include a thorough ovarian reserve assessment through AMH testing, along with thyroid and blood sugar checks to identify any hormonal or metabolic issues. She also recommends comprehensive reproductive health screenings to ensure the body is well-prepared for conception.

Dr. Batchu Sowdamini, Senior Consultant in Obstetrics & Gynecology at Apollo Hospitals, Vizag, adds, “After 35, egg count decreases and the chances of genetic abnormalities increase. Being aware of these factors allows women to plan ahead and minimize risks.”

Managing Risks and Debunking Myths

Pregnancy after 40 often carries the label of being “high-risk,” but that doesn’t mean it’s unsafe. With consistent medical monitoring and planning, many women have smooth, healthy pregnancies.

Dr. Bhatt clarifies, “Is 40 too late? Absolutely not. But it’s not the same as conceiving at 25 or 30. Planning, monitoring, and having the right medical support are key.”

Common misconceptions—like assuming good health eliminates age-related fertility decline or that IVF guarantees success at any age—can lead to disappointment. The success rate of IVF decreases as women age, and while medical science offers incredible options, understanding limitations is essential.

Dr. Sowdamini notes that pregnancies in the early 40s often come with higher chances of fetal growth restrictions, chromosomal anomalies, and conception difficulties. She recommends early planning, ideally before 35, to improve outcomes and lower risks.

Lifestyle Choices That Support Healthy Conception

Medical experts agree that lifestyle choices play a defining role in fertility and pregnancy outcomes. A healthy body creates a favorable environment for conception and fetal development.

Dr. Bhatt recommends a well-rounded, mindful approach that includes:

1. A balanced, Mediterranean-style diet rich in antioxidants and lean proteins
2. Regular physical activity to maintain a healthy weight
3. Quality sleep and stress management practices like yoga or meditation
4. Limiting exposure to environmental toxins

She emphasizes that consistent routines and emotional support form a strong foundation for a healthy pregnancy journey.

For women planning conception after 35, early medical testing is critical. Dr. Bhatt recommends the following:

1. Baseline fertility testing: AMH, FSH, and estradiol levels
2. Antral follicle count (AFC) through ultrasound
3. Comprehensive health screening for overall wellness
4. Partner evaluation and genetic counseling to identify potential hereditary risks

Dr. Sowdamini also emphasizes the importance of egg or embryo preservation for those delaying pregnancy. This provides an added option for conception later in life with reduced risk.
